When receiving care at MedWest-Swain, you will need to provide the following information to our registration team:
Your full name
Date of birth
Social Security #
Telephone number with area code
Insurance coverage plus a copy of all insurance cards
Emergency contact information
A copy of any information (orders, etc) that your doctor has given you
This information, which is handled confidentially, ensures that accurate data is provided to those involved in your care.
Registration Hours: 6:30 a.m.-11:00 p.m. 7 days per week.
Registration is completed by the Emergency Department Staff between 11:00 p.m. and 6:30 a.m.
Hours of Operation: 8:00 a.m. - 6:00 p.m., Monday - Friday.
Your physician office will normally schedule your test for you. If you do not have an appointment date and time, you may contact Centralized Scheduling at 1-855-298-3003.
When You Arrive
On the day of your visit, please be sure that you sign in at the Reception Area upon arrival. The Receptionist/Switchboard Operator will direct you to the appropriate location to be registered as well as to the appropriate department within the hospital. Please bring the following items with you:
At least one form of identification
Your insurance card(s)
Insurance referral forms (as required by your Insurance company)
Any written orders from your physician
