Not yet ready for Hospice services
Services Provided:
Palliative Care Nurse will assist with symptom management
Music Therapy offered to patient and family
Volunteer services available for respite and support
Chaplain services by hospitce trained personnel
Bereavement services available to the family
Inpatient Palliative Care
Provided at no additional charge to the patient
Provides additional supportive services while in hospital
Provides patient/family with a contact person should they become hospice appropriate
The Referral Process
MD order obtained for hospice/palliative care consult
Liaison nurse reviews the medical record and meets with the patient, family and case manager
If appropriate, palliative care initiated
If hospice appropriate, referral made
